Volunteers Needed for Health Care Homes Program

May 4, 2023

The Minnesota Department of Health’s (MDH) Health Care Homes (HCH) Program has several openings on its advisory committee.

The committee, which advises the health commissioner, is made up of 15 to 16 members with representatives from healthcare professions, HCH clinics statewide, health plans, employers, academic researchers, quality improvement, and consumer/patients. Members are expected to be active participants, attend meetings, and represent their constituents. The committee meets virtually three to four times a year. If in-person meetings resume in the future, a virtual option will be available. The advisory group has two work groups that members may join if they choose. Additional information on the advisory committee can be found online.

Applications can be submitted via the Secretary of State’s Office website
