7-OH, Kratom Clinical Guidance

The Minnesota Medical Association (MMA) and the Minnesota Society of Addiction Medicine (MNSAM) developed this clinical resource in partnership to support Minnesota clinicians caring for patients who use concentrated 7-hydroxymitragynine (7-OH) and related kratom-derived products.

These products can produce opioid-like effects and may be associated with dependence, withdrawal, and other serious adverse effects. At the same time, the evidence base for treatment remains limited, product labeling and potency can be unreliable, and clinicians may encounter significant variation in patient presentation.

The guidance package was developed to provide a practical, Minnesota-specific framework for assessment and buprenorphine initiation across inpatient, outpatient, and at-home settings. It is intended to support clinical decision-making, not replace clinical judgment, local policy, toxicology consultation, or emergency care. Because treatment evidence for 7-OH remains limited, this resource incorporates established opioid use disorder practice and the experience of Minnesota clinicians caring for this emerging patient population.
