Virtual Summit on Pediatric to Adult Health Care Transition to be Held June 16

Gillette Children's and the Minnesota Department of Health will host a virtual Pediatric to Adult Health Care Transition Summit on Monday, June 16 from 8:30 am to 3:50 pm.  

The summit will offer participants insights into enhancing programs and overall state practices regarding pediatric to adult health care transition (HCT). This progress is urgently needed to support the growing number of youth with special health needs aging into adulthood.  

After attending this summit, participants will be able to:  

  • Describe the current state of HCT outcomes and ongoing efforts to improve them. 

  • Utilize current services and resources available from state and local organizations to improve HCT processes in their own setting. 

  • Describe mental health resources and support available for youth and young adults. 

  • Apply learnings and best practices from a multi-department HCT program that was incorporated into a large health system. 

The summit is open to those interested in advancing pediatric to adult HCT. This includes pediatric/adult clinicians from any setting, young adults and adults living with childhood-onset conditions and their caregivers, family advocacy and professional organizations, insurers, policymakers, health systems and health administrators, and more.
