Social Drivers of Health in MN to be Discussed at Virtual Event

As part of the Minnesota Health Equity Community of Practice (CoP), Stratis Health will present key findings from its project on health-related social needs assessments and referrals on November 15 from 1 to 2 pm.  

The project goal was to create new ways of identifying social needs through screening and designing closed-loop referral processes to ensure patient needs are met. 

Stratis will also share information about Culture Care Connection, an online learning and resource center that supports clinical and non-clinical healthcare professionals to reduce health disparities and promote health equity. 

This virtual meeting is open to all physicians, health professionals, and leaders from Minnesota health systems, medical groups, and medical associations with an interest in health equity. To register, click here.  

The CoP is a peer network dedicated to advancing health equity and addressing the significant racial and ethnic health disparities that persist throughout Minnesota. Health equity leaders and professionals come together quarterly to exchange expertise, network, and discuss issues and priorities.
