Beginning July 18, the MMA will host a series of noon-time sessions called “Project ECHO: Navigating Transition for Youth with Medical Complexity to Adult Care.”
The purpose of this project, which is funded by a grant from COPIC Medical Foundation, is to develop a sustainable and collaborative knowledge-sharing community to improve the transition from pediatric to adult care for youth with medical complexity.
“Thanks to clinical and technological advances, many children with medical complexity now live into adulthood,” said Tori Bahr, MD, associate medical director of pediatrics at Gillette Children’s Specialty Healthcare, and the project’s medical advisor. “However, these patients face numerous practical hurdles in moving from pediatric care to adult providers. The project aims to improve the competence and confidence of Minnesota adult primary care clinicians to manage youth with complex conditions, thereby increasing the capacity and number of such clinicians willing to accept new patients.”
The online sessions begin July 18 and will continue through early 2023. A summit that will summarize much of the learning in the sessions will take place in March of 2023.
