Health Care Home Rule Expands Emphasis on Population Health, Equity

The Minnesota Department of Health (MDH) Health Care Homes (HCH) team announced on October 10 that a rule amendment that expands the program’s emphasis on population health has been approved. 

Along with population health, the new advanced certification levels created by this rule expand the program’s emphasis on addressing social determinants and enhancing health equity.
