previewing the Minnesota Department of Human Services’ new Medicaid Encounter Alert Service (EAS) is now available free of charge. This new service is available to any physician or practice, but may be particularly valuable to practices who are part of a Medicaid Integrated Health Partnership (IHP), ACO or other value-based payment program.
The EAS will enable physicians and other providers to receive alerts for Medical Assistance beneficiaries who have been admitted, discharged, or transferred from a hospital, emergency department, or other care setting. The state has selected Audacious Inquiry (Ai) as the EAS vendor for Minnesota.
The EAS is designed to support care coordination, reduce hospital readmissions, and enable improved management of cost-effective care delivery. The EAS will help physicians and other providers identify where patients are receiving care throughout the state, regardless of electronic health record. The service is being launched, initially, as part of the state’s Integrated Health Partnership value-based care delivery initiative, and is funded through the State Innovation Model (SIM) grant.
