Virtual Roundtable to Focus on the Transition from Pediatric to Adult Healthcare 


An online roundtable discussion titled “Pediatric to Adult Healthcare Transition Roundtable: The Medically Complex as a Model of Systems Barriers and Potential Solutions,” will be held June 14 from 1 to 4 p.m.  

At the end of the discussion, attendees should be able to: 

  • Identify systemic barriers to transition in their own practice. 

  • Develop language to have conversations with their health system to put in place a process to support transition to adulthood for patients with complexity or rare diseases. 

Scheduled panelists include: Nathan Chomilo, MD, medical director, Minnesota Medicaid and MinnesotaCare; Kurt Elting-Ballard, MD, MPH, regional medical director, Minneapolis Region and Abbott Northwestern Market; Gretchen Karstens, MD, pediatrician, St. Luke's Pediatric Associates; Tom Kelly, vice president marketing and innovation, UnitedHealth Group; and Pat Lang, PACER Center.
