[MMA News Now, May 24, 2012] Reducing Avoidable Readmissions Effectively (RARE) will host a free webinar on medication management in both ambulatory care and skilled transitional care units on June 19 from noon to 1 p.m.

The webinar will cover: a pilot project at Park Nicollet Health Services to improve transitions of car by evolving the role of the pharmacist; and how a consultant pharmacist and provider pharmacy can expand their roles and help reduce hospital readmissions by providing key services to patients during the transition from hospital to skilled nursing facility and from the facility to home.

Speakers will include Alison Knutson, a pharmacist at Park Nicollet; and Joe Litsey, the director of consulting services at Thrifty White Pharmacy.
