Diabetes Pilot Project Seeks Physician Participation

[MMA News Now, March 6, 2014] The AMA and the YMCA of the Greater Twin Cities are teaming up to help metro-area physicians and their pre-diabetic patients.

Specifically, the AMA is seeking primary care practices to join a six-month pilot project designed to:
• Increase education and awareness of pre-diabetes and promote screening by physicians of those at risk
• Increase physician referrals of people at risk for diabetes to the Y’s Diabetes Prevention Program (DPP)
• Create a feedback loop so the patient’s experience at the YMCA becomes integrated into the physician’s care plan for the patient

The AMA is focusing on the linkage between physicians’ practices and the local YMCA. The two organizations already are collaborating to increase the number of Medicare participants in the YMCA’s Diabetes Prevention Program.
 
The YMCA’s DPP is based on the Centers for Disease Control and Prevention’s National Diabetes Prevention Program.

we are looking at community connections, esp with latino families. any pediatric component?
