Project ECHO logoNavigating Transition for Youth with Medical Complexity to Adult Care

With a grant from the COPIC Medical Foundation, the MMA and the MMA Foundation hosted a series of noon-time online sessions intended to develop a sustainable and collaborative knowledge-sharing community to improve the transition from pediatric to adult care for youth with medical complexity. Thanks to clinical and technological advances, many children with medical complexity now live into adulthood. Yet they face numerous practical hurdles in moving from pediatric care to adult providers. This project aimed to improve the competence and confidence of Minnesota adult primary care clinicians to manage youth with complex conditions, thereby increasing the capacity and number of such clinicians willing to accept new patients. 
 
The power of the ECHO hub-and-spoke model is in its capacity to leverage technology (Zoom-like sessions) to expand expertise and knowledge without regard to geography or practice type. As described by Project ECHO, “during an ECHO session, participants (the spoke) present real (anonymized) cases to the specialists (the Hub)—and each other—for discussion and recommendations. Participants learn from one another, as knowledge is tested and refined through a local lens.”
